Student Internship Agency Contract

This contract will verify that _______________________________ (name) will be completing an internship at _______________________________________(location) beginning ________________(Month) _______(Date) , 200__ and terminating _______________(Month) ______(Date) , 200__. It is understood that the above named person will complete a minimum of 400 hours in the internship program and will receive nine credit hours toward graduation. The student also must complete a special project on his/her own with guidance from the agency supervisor. This project should provide a learning opportunity for the student and should make a contribution to the agency sponsoring the internships.
